POSITION SUMMARY  

This position provides data entry and analysis support for the Health Home Program. The position analyses data and assists with maintaining all internal and external data management systems.  The position also is responsible for monitoring Medicaid status of all clients involved in program, collecting, monitoring/maintaining internal tracking logs, CMART reports, error reports, case openings, case closures, bottom ups and top down assignments, and submitting weekly data report to Data Manager 

 

ESSENTIAL FUNCTIONS

·         Data entry into multiple clinical databases, including TREAT, Excel, eClinicalWorks, and other Health Homes systems

·         Uploads and download information from all of the Lead Health Homes electronic systems and ensures data matches internal data base

·         Ensures reports to the lead are accurate, complete and timely

·         Monitors and reports clients’ Medicaid status

·         Provides EHR support including login-in, trouble shooting and training

·         Collects, monitors/maintains internal tracking logs, CMART reports, and error reports

·         Responsible for all case openings, case closures, bottom ups and top down assignments, and submitting weekly data report to Data Manager

·         Compiles information for special reports and maintains contact with other departments on related projects

·         Works collaboratively on quality assurance / quality improvement activities

·         Maintenance of data-related records

·         Data analysis and reporting

·         Responding to standard requests for information regarding data

·         Compiling information for special reports and maintains communication with other departments on related projects

·         Performing other duties as required to support the department and its mission

EDUCATION/EXPERIENCE:

High school diploma / GED and 4 years of related work experience.  Associate’s Degree and two years related experience, or a combination of education and experience that would provide for the following knowledge, skills and abilities:
