Hudson River HealthCare (“HRHCare”) is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. HRHCare is currently seeking a full-time Registered Nurse to join our Patient Care Contact Center in Coram.

The Registered Nurse provides nursing triage services (assessing, prioritizing and determining disposition) of patients calling into the regional call centers to ensure prompt identification of patients with high risk conditions. May additionally provide other telephone based nursing services such as education, medication management and health screening/wellness assessments.

Essential duties and responsibilities of the Patient Care Contact Center Registered Nurse:

  • Knowledgeable of HRHCare telephone triage algorithms for common conditions and applies correct assessment education and disposition based on standardized template
  • Knowledgeable about telephone based clinical assessment skills and techniques
  • Reviews patient’s medical history during telephone encounter and validates and documents pertinent history in EHR and determines urgency of present complaint based on assessment and history
  • Uses appropriate communication techniques in a calm and clear manner to elicit appropriate information from patient during triage encounter
  • Creates appropriate appointments for patients as indicated by triage protocols and appointment templates
  • Documents all pertinent information, including assessment, interventions and disposition in the EHR, including follow up telephone encounter to provider/clinical team
  • Maintains call volume and length of call benchmarks for call handled by Triage nurse
  • Works collaboratively with local providers at the health centers to ensure continuity of care
  • Competent in technology usage including telephones, computers and software products as necessary
  • Provides safe clinical triage and disposition of patient’s symptoms with appropriate clinical knowledge and judgement as documented by telephone encounter review and call auditing per policy

Minimum Education Requirement:

•Associate’s Degree

Preferred Education Requirement:

•Bachelor’s Degree

Minimum Work Related Experience:

•2-4 years of experience

Minimum Requirement for Licensure/ Certification/Registration:

•Registered Nurse - NYS

•CPR Certification

•Infection Control Certification

Qualified Candidates:

•Ability to communicate in English, both orally and written

•Possess supervisory experience

•Knowledge of nursing standards and competence in nursing skills

•Ability to triage patients calls or walk-ins as necessary

•Strong commitment to public and community health and has the ability to work with individuals from a variety of ethnic and socioeconomic backgrounds

•Bi-lingual (Spanish speaking) strongly preferred
